Opening Script
"Welcome back. Before we continue, I want to check in on what we worked on last session and see how you've been doing since then. This helps us make sure the processing is holding and identify if anything else needs attention."
Reevaluation Steps
General Check-In
"How have you been since our last session? Have you noticed anything different - thoughts, feelings, dreams, body sensations, or any new insights about what we worked on?"
Client Report:
Access the Target
"Let's check on the memory we processed last time. When you bring up that memory now, what comes to mind? Just notice what image, thought, or feeling arises."
Check SUD
"On a scale of 0 to 10, how disturbing does this memory feel to you now?"
Check VOC
"And how true does the positive belief '[PC]' feel now, on a scale of 1 to 7?"
Based on Reevaluation
SUD = 0, VOC = 7
Target is complete. Move to next target in treatment plan (Past, Present, or Future).
SUD > 0 or VOC < 7
Resume processing from Phase 4 or 5. There may be new channels or blocking beliefs.

Instructions for Client
- - Fill this out when you notice disturbance between sessions
- - Trigger: What happened right before? (situation, person, place, sensation)
- - Image: What picture or scene came to mind?
- - Cognition: What negative thought about yourself arose?
- - Emotion: What feelings came up? (fear, anger, sadness, shame, etc.)
- - Sensation: Where did you feel it in your body?
- - SUD: Rate the disturbance from 0 (none) to 10 (worst)
- - Bring this to your next session so we can discuss together
Reminder: After noting disturbance, use your resources (Safe Place, Container, Breathing) to return to calm. You don't have to process this alone - we'll work on it together in session.
